TEREX TELELECT, INC. CERTAIN TLM SERIES AERIAL DEVICES WHICH USED MOUNTING PLATE 471499 INSTALLED ON UTILITY TRUCKS FROM DECEMBER 2002 THROUGH NOVEMBER 2007. THE PLATE CAN DEVELOP A CRACK AT THE BASE OF THE PEDESTAL TO THE TRUCK FRAME.

IF THE MOUNTING PLATE FAILS, THE BOOM AND PLATFORM MAY FALL, POSSIBLY RESULTING IN SERIOUS INJURY OR DEATH.
