Official US government data: the federal lab certificate records (updated every quarter) and Original Medicare claims of labs, doctors and suppliers (yearly). Segments are our grouping: independent and reference labs include every lab of a listed lab company; physician office labs bill under the doctor. Tests reported by labs are yearly totals for labs that run more than simple tests. Medicare tests that hospitals bill for their own outpatients are paid under a different part of Medicare and are not included. Lab test codes are shown by number with their category, because the official code descriptions are licensed.

There are 301,760 active labs with a federal lab certificate (as of April 2026): 116,558 physician office labs, 7,942 independent and reference labs, 9,404 hospital labs and 167,856 other labs such as pharmacies, nursing homes, clinics and dialysis centers. 245,568 of them hold a certificate of waiver, which allows only simple tests such as glucose meters and rapid strep tests.
By number of labs: physician office (116,613), other (39,325), pharmacy (22,815), nursing home (14,749), home health agency (13,046), assisted living facility (11,039). Independent labs are few but run most tests: they reported 3.55B tests a year, against 4.48B for hospital labs.

Counts are lab certificates; collection sites that do not test are not labs.
Original Medicare paid $10.04B for 451.5M lab tests billed by labs, doctors and suppliers in 2025. Independent and reference labs billed 321.9M of them (71%). The largest test categories were chemistry (183.3M tests), specimen collection (101.1M tests), anatomic pathology and cytology (39.8M tests), hematology and coagulation (38.0M tests).
